    Chartwell Wish of a Lifetime will review your application and may contact you to schedule a screening call or request additional information.

    If your wish is selected to be granted, you will be asked to complete the following forms:

    • Liability Release and Authorization Form.
    • Medical Verification Form.

    If your application progresses to the next stage, our team will coordinate the details of the experience, such as ticket bookings, accommodations, transportation, and all other necessary arrangements.
